Doesn't the bill say at the bottom if the tip is included or not? Or are you talking about the places where you're lucky to get your bill written on a piece of paper?

No, only rarely. You simply cannot get someone to work for the piddling amounts paid in the US and maybe the UK. The service is part of the cost of the food and only small roundings up are appropriate. It would be against the law to pay a waiter $3-6 an hour and tell them to hustle the rest in tips. They also get health and pension coverage.
Those ones that don't give you a real receipt are tax dodgers and up to a couple of years ago you, too, would get a fine of euro 500 for not getting a receipt while the shop would pay euro 1500.

My teachers were quite good, and the education system I was in was classed as the top level in the whole of Australia...... in it's day.
It's just in recent years that the standards of 'teachers' (worldwide it seems) have slipped abysmally. We have teachers in the system now who can't even spell FGS!!!
Then we have shining examples like you who go off at someone, quite incorrectly, because you couldn't sort out the attributions and read who was saying what.
Do you check your pupils schoolwork with the same lacklustre attitude and lack of attention to detail?
